PRECONFERENCE: POPULATION HEALTH BOOT CAMP
|
This intensive session is designed for professionals working in various aspects of health care who wish to improve their knowledge and understanding of the tenets of population health, and its application in real-world settings.
Quality improvement, care coordination, and prevention and wellness are among the key cornerstones of the new health reform legislation. The preparatory readings and preconference workshop session will provide participants with an overview of population health management and the principles of care coordination; explore changing models of care for chronic illness prevention and chronic care management; and discuss the value proposition for practice-based approaches to chronic illness prevention and chronic care management.
The program will be taught by full-time faculty from the Jefferson School of Population Health.
