PRECONFERENCE I: THE VOICE OF THE PATIENT IN POPULATION HEALTH

This engaging, interactive preconference will share the latest research and perspectives on the voice of the patient in health care. The most recent evidence on patient and family engagement will be presented by the team at the O'Neil Center at GetWellNetwork, whose mission is to advance the science of patient and family engagement.
The day kicks off with a keynote presentation by Michael O'Neil, CEO and Founder of GetWellNetwork, who was diagnosed with non-Hodgkins lymphoma at age 28 and then dedicated his life's work to leveraging technology to engage people in their care. The innovative work at the O'Neil Center includes the application of the Interactive Care Model™, and the new Person Engagement Index©.
Join Michael and his team from the GetWellNetwork, Inc. and the O'Neil Center as they discuss ways to advance the integration of patient and family engagement into health care delivery.
Session Overview/Objectives
- Relate a case study about a person's care experience to the needs of the health caresystem to improve population health.
- Identify the development and uses of a Person Engagement Index (PEI) to assess a person's capacity to be engaged in their care journey.

PRECONFERENCE 2: The POPULATION HEALTH ALLIANCE FORUM 2017 -- STAKEHOLDERS UNITED FOR POPULATION HEALTH

The Population Health Alliance Forum 2017 -- Stakeholders United for Population Health will be held on Monday, March 27th from 8 am -- Noon in conjunction with the Population Health Colloquium in Philadelphia, PA.
This session will include exciting presentations and panels on industry wide and focused topics to include population health future trends and funding under the new administration, senior health and life sciences implications for population health. The session will close with a panel represented by the Population Health Alliance Board of Directors called "PHA Looking Glass" which will give you a view into the strategic initiatives supported by PHA.
